Nothing to Sneeze At: Understanding Allergic Rhinitis

If any individual has chronic symptoms of nasal congestion, sinus problems, or frequent colds, they should be evaluated for allergic rhinitis. Symptoms include sneezing, nasal congestion, and a runny nose. Tearing and itching can affect the eyes as well as itching in the throat. Ear fullness or pain, along with facial pressure, may be due to sinus involvement. Headaches, fatigue, and irritability can be non-specific symptoms.

Seasonal allergic rhinitis is worsened by pollen and outdoor molds, especially during the spring and fall. Airborne mold allergies are increased in areas with high humidity (as we have in Houston) and are responsible for many cases of allergic rhinitis.

Management includes nasal irrigation to clear the passages of thick mucus secretions either with store-bought sprays, or home preparations of a pinch of baking soda and salt in warm water with a bulb syringe three or four times a day. Antihistamines, either orally or by topical nasal sprays and eye drops are effective for sneezing and runny nose, but not for nasal congestion. Topical decongestants are best for this, but should not be used for more than 3-5 days at a time in order to avoid rebound congestion. Pseudoephedrine is a commonly used over-the-counter medication for congestion. Intranasal steroids improve all the symptoms of allergic rhinitis including nasal congestion. An anticholinergic nasal spray can reduce symptoms of a runny nose. Intranasal cromolyn sodium is available without a prescription and has the least side effects of all the medications used for allergies and may be used for maintenance or preventive therapy. Montelukast, previously indicated only for asthma, has been approved for seasonal rhinitis.
